Job Description:
A company is looking for a Remote Case Manager, RN.
Key Responsibilities:
Monitor utilization patterns and facilitate appropriate health care
delivery for selected members
Perform comprehensive assessments of members' health status,
including medical records and support needs
Collaborate with healthcare teams to develop and coordinate
treatment and discharge plans
Qualifications:
RN or LPN with a North Carolina license required
Prior telephonic case management experience is a plus
3+ years of inpatient care experience
Critical care experience, including med-surg and ICU
Must reside in the EST time zone
